Throughout this Waiver & Release, Atlanta Mission, In consideration for the opportunity to be interviewed and/or photographed in connection with Atlanta Mission, I agree to the following: I hereby give Atlanta Mission the absolute right and permission to copyright and/or publish, or use photographic portraits or pictures of me, or in which I may be included in whole or in part, or composite or distorted in character or form, in conjunction with my own or fictitious named, or reproductions thereof in color or otherwise, made through any media at their studios or elsewhere, for art, advertising, trade or any other lawful purpose whatsoever at any time.
I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith, or the use in which it may be applied.
I hereby release, discharge, and agree to save Atlanta Mission from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur to be produced in the taking of said pictures, or in any processing tending towards the completion of the finished product.
I have read the above Waiver, Release, and Indemnity Agreement and understand that by signing below, I have given up substantial rights.
I, the undersigned parent or legal guardian of the child listed above, have read the above Waiver, Release, and Indemnity Agreement and agree to its terms on behalf of my child and myself. I understand that by signing below, I am giving up substantial rights on behalf of my child and myself.
